Axx seems so limited by the fact that it only has 16 possible values for each direction, so it can't do really long fades. I have seen people using long strings of consecutive A01s, but that seems so cumbersome. Why not just add two more effects, say, Mxx and Nxx, that volume slide up and down, respectively, but with xx being the value in the range 0 - 255 for each?

Also I think the transpose effect should be done in Pxx's style (?80 = Reset, ?87 = a fifth higher, ?74 = an octave lower), this way it can save one more slot and can reach more notes (unless you are telling me that K20 to KFF are reserved for other effects)

It is tedious and time-consuming in an OHC situation, yes. And it is longer than just entering an effect and re-using an already existing pattern. I just think it would be nice to have the option to do the latter.
And yeah, come to think of it, nicetas is right: having Kxx work like Pxx would be even more effective than my suggested method.

You do realize you'd have to make a new pattern anyway so that the effect wouldn't affect every instance of the pattern, right?
Not necessarily. Say you have a pattern ending with K81 (which would transpose up one semitone), but you only want to transpose the next pattern the first time this one plays, but not the second time, then all you'd have to do is use K80 on line one of the next pattern to reset the effect and you're set.
Perhaps a phase reset?
As in, start the channel over like you stopped it and started it again. Useful for lining up unison chords.
Maybe even a phase set effect.
