There's an English manual floating around somewhere, and I believe there's a way to replace the Japanese text/icons, but it's been ages since I tried it and I can't remember the details. It can make NES sounds, but it's general purpose, not specifically an NES thing.
Edit: Delete japanese.ico to get English menus. There's an example project in Samples that does an NES imitation.
