The module comments explain it better than I do, but I'll say it here too.
All this Namco instrument is is a simple modulating sine, but then after the volume envelope drops all the way, it starts playing on its own.

You're calling a lot of undefined waves in your wave envelope, for starters.
Simply pressing "enter" in the volume envelope MML field seems to fix the issue though.

I did edit the wave envelope to only use the defined waves but it still glitched up. Then I simply made a new volume envelope and it sounds fine to me now...
